The CD74-ROS1 fusion is a genetic rearrangement that occurs in some cancers, particularly in non-small cell lung cancer (NSCLC) and other malignancies such as brain tumors and hematological malignancies. This rearrangement results from the joining of the CD74 gene with the ROS1 gene. Normal CD74 and ROS1 are membrane proteins that are involved in immune responses, cell signaling and growth, respectively. The fusion leads to the production of a chimeric kinase protein that has oncogenic properties, which means the CD74-ROS1 fusion protein can promote tumor growth and survival. Several types of CD74-ROS1 fusions have been reported in cancer patients. The known breakpoints of CD74-ROS1 fusion gene are exon 3 of CD74 and exon 34 of ROS1 (C3R34), exon 6 of CD74 and exon 32 of ROS1 (C6R32), exon 6 of CD74 and exon 34 of ROS1 (C6R34), exon 7 of CD74 and exon 32 of ROS1 (C7R32). Cancers with CD74-ROS1 fusions, can be treated using specific tyrosine kinase inhibitors (TKIs) that inhibit the activities of the CD74-ROS1 fusion protein. So far, the commonly used drugs for ROS1-positive cancers include crizotinib, entrectinib. However, some patients may develop resistance to certain TKIs, so it is necessary to discover new TKI candidates to improve efficacy and overcome resistance mechanisms associated with existing therapies.
